Parish Survey 2014

Dear Parishioner
The Parish Pastoral Council is conducting an anonymous survey to gather ideas about what works well at Holy Trinity Parish and what we can do better.  We would like to ensure that we have a vibrant and spiritual community that meets the needs of all parishioners.  We are also developing a vision and mission statement for the parish and would like to ensure that your views are reflected.

 

INSTRUCTIONS FOR DOWNLOADING AND COMPLETING SURVEY

2. You may need to wait for the page  to load properly.
3. Please click on ‘DOWNLOAD’ and complete form using Adobe Acrobat Reader.
4. Once completed, return to us by clicking on ‘SUBMIT FORM’ which appears on the top right hand corner.
